Özet
Aim: Immune thrombocytopenia (ITP) is an acquired, quantitative, consumption-related platelet disorder and its diagnosis requires exclusion of other causes of thrombocytopenia. The relationship between time to response to treatment and splenectomy was investigated in this study, in 21 cases of ITP in whom steroids were started as initial therapy. Methods: The medical records of twenty-one cases of ITP who were followed-up at the Hematology Outpatient Clinics between 2009-2012 were reviewed. All patients were given methylprednisolon at an initial oral dose of 1 mg/kg. The platelet counts at days 0, 3, 7, 14, and 21 were evaluated by hemogram and peripheral smears. Patients who were unresponsive to steroids or relapsed were treated with splenectomy. Results: Twelve (57%) of the 21 cases were females and 9 (43%) were males. Nine of the 21 patients (43%) underwent splenectomy. The percentage of patients who had a platelet count over 100 109/L on day 14 was 75% and %22 in patients who did not undergo splenectomy and who underwent splenectomy, respectively. All patients (%100) who did not undergo splenectomy but only 2 patients who underwent splenectomy had platelet counts over 30x10 9/L on day 7. All patients with a platelet count lower than 30x10 9/L on the day 7 underwent splenectomy. Conclusions: These findings have shown that early response to steroids in ITP patients may have predictive value for future requirement of splenectomy.
