Abstract

In this article, we present a 57-year-old male patient who underwent the first successful multibranched aortic stentgraft implantation in Turkey for total endovascular repair of pararenal aortic aneurysm. The patient was deemed unfit for open surgery due to his comorbidities and Cook Zenith® t-Branch™ aortic endograft implantation was planned. A staged procedure was preferred to allow time for collateralization to the spinal cord and prevent paraplegia. In the first procedure, Cook Zenith® t-Branch™, all visceral side branch covered stents, Cook Zenith® Unibody™, and ipsilateral limb graft were implanted. All target vessels were successfully connected without evidence of occlusion. In the second stage, contralateral iliac leg was placed. No organ ischemia or renal function deterioration were observed. Branch graft patency was 100% on sixth month computed tomographic angiography examination with a minimal type 2 endoleak which did not expand the aneurysmal sac.
